Ashley Treadwell, IBCLC, currently accepts the following insurance for any visit (home, office in both locations): Aetna, Blue Cross Blue Shield, Cigna, United, PacificSource or TriCare. Ashley can accept the following insurance at Canyon Medical Center SW office location: Moda, OHP, Providence. If you are self pay, Ashley will provide a superbill for you to submit to your insurance for possible reimbursement if your plan has Out of Network benefits. Breanna Grishman, IBCLC, is currently in-network with: Aetna, Blue Cross Blue Shield, Cigna, MODA, Pacific Source, Tricare and United Healthcare. Any other insurance will be cash pay. The only insurance lactation is UNABLE to take during home visits are: Kaiser, OHP and Providence
